2026 Research Is Pointing to a New Root Cause of POTS. And Why You Were Never Imagining It

Women are reporting change in symptoms they were told they would have for the rest of their lives, after finally addressing a tissue no POTS drug has ever been designed to touch.

Fifteen years of my working life have gone into one layer of tissue. The reason is my younger sister.

She was nineteen. Second year of her degree, and by that October she could not stand up in a lecture theatre without the room draining white. She was diagnosed inside the year, only because I knew which test to ask for. Before that she was told she was doing it to herself.

She did not finish the degree. She moved back into her childhood bedroom and she is still in it. The group chat from school went quiet by the second winter. She is thirty and nobody has given her a reason.

A faded family snapshot on a British seaside promenade, with a handwritten red arrow reading My sister, before POTS.
Summer 2015. She collapsed in a lecture theatre that October.

The specialty was a decision, not an accident. I wanted the room these women get assessed in.

Most of the people in it had made their minds up before she sat down. There is a vocabulary for a young woman with a racing heart and clean results, none of it in the literature. Anxious. Deconditioned. A hypochondriac. One of those things that happens to young women.

And I had nothing to put against it. Propranolol, midodrine, fludrocortisone, later ivabradine. That is the shelf, and it is the same shelf my sister was handed at nineteen.

That is beginning to change, and not because of a new drug. Somebody pointed the measurement somewhere other than her heart.

WHAT THE 2026 SCANS REVEALED

In January a team took fourteen women with POTS and fifteen without it and asked them to do one thing. Stand up. No tilt table set to catch a heart rate. Just stand, while the blood in the lower leg was measured.

Same posture. Same measurement. Two different bodies.

The standing calf of a woman with POTS held twenty-nine per cent more blood than the calf of a woman without it.

A leg is not a bag of blood. It is muscle held in compartments by a continuous sheet of connective tissue, and no cardiologist examines that sheet and no POTS drug was designed to affect it.

It is called fascia.

01 The POTS Standing-Volume Study
Fourteen women with clinically diagnosed POTS and fifteen without it, measured while standing. Calf venous volume was 29% greater in the POTS group, and the leg took almost twice as long to refill.

It is the same shelf my sister was handed at nineteen.

WHAT FASCIA ACTUALLY IS

It is connective tissue running through the body as one continuous layer, directly under the skin, wrapping every muscle, from the skull to the pelvis without a break, with pain nerves running through it in large numbers.

For most of the last century students were told to peel it off the muscle and throw it away, because it was in the way of the interesting biology.

That is why it has never come up in an appointment. Not because her cardiologist missed something, but because nobody thought the tissue did anything.

It is not one sheet either. It is several, stacked, and they move across one another every time the body moves.

What they slide on is a thin film of water, held by one long molecule. Hyaluronan. The same material that makes joint fluid slippery.

Anyone who has washed up knows it. Two wet plates slide. Let the water go and the same two grip.

Evidence Peer-Reviewed Findings
02 The Hyaluronan-In-Fascia Research
Researchers in Padua went looking for hyaluronan in human fascia and found a distinct layer of it between the fascia and the muscle, and more in the loose tissue between the sliding sheets.
Stecco et al., Surgical and Radiologic Anatomy, 2011

Skin, fat, then the sheet that wraps every muscle. A tilt table reads none of it.

EVERY SYMPTOM FINALLY MAKES SENSE

Once the tissue is in the picture, what she carries alongside the heart rate stops looking like a separate list. Muscle pain is reported by 84 per cent of POTS patients. Not a dozen problems. One problem in a dozen places.

01
The band of ache across the neck and shoulders
The symptom. A dull weight from the base of the skull out to both shoulders, there by mid-morning, worse in a warm room, gone minutes after sitting down. In orthostatic disorders clinicians call that shape a coat hanger.

Why it happens. One sheet of this tissue covers the neck and shoulders, in layers that slide every time an arm lifts. When they stop sliding, the arm is working against a layer that has stopped giving.
02
The legs that ache from standing, not from walking
The symptom. Twenty minutes at a counter or in a queue and the thighs and calves begin to burn, when an hour on her feet moving did nothing last week.

Why it happens. Every muscle in the leg sits in a sleeve of this tissue, built around movement. Walking works water between the layers. Standing still does none of that.
03
Why getting up after a long sit is the worst of it
The symptom. Two hours in a chair, a long drive, a meeting that overran, and then standing is worse than it would have been at the start.

Why it happens. A sliding surface needs movement to keep water between its layers. Stillness draws it out, and the first movement afterwards has to break the grip.
Two wet plates sliding across each other.
Wet, two plates slide apart in the hand. Dry, the same two grip and stay gripped.

Same posture. Same measurement. Two different bodies.

HOW THE FASCIA GOT THIS WAY

This is the part her own doctors waved away.

POTS usually has a before and an after. A virus she never properly got over. Glandular fever in her teens. A COVID infection, and a life that was one shape in the spring and another by the autumn. In three to four cases out of ten, symptoms began within three months of an infection.

She was told the timing was a coincidence. That the illness was behind her and the rest was in her head.

Every one does the same thing on the way through. The body meets a serious threat by scaling back supply to the tissues it does not need that week and sending it to the heart, the brain and the lungs.

Connective tissue is a long way down the list of what gets protected in that moment, and in some people the supply never fully comes back.

Nobody knows why it resumes in most people and not in others. What is not in question is the direction. An under-supplied sliding surface does not recover on its own.

Two panels, the same woman, the connective tissue dry and gripping on the left and supplied on the right.

On the left, the stacked sheets that wrap every muscle have lost the film of water they slide on, so they grip instead of glide. That is the band across the neck and shoulders, the thighs that burn at a counter, and the stand-up after two hours in a chair. On the right, the same sheets supplied again, the one state nothing on her shelf was aimed at.

WHY THE STANDARD MENU COULDN'T TOUCH THIS

Propranolol and metoprolol slow the heart. Ivabradine slows it another way. Midodrine squeezes the vessels. Fludrocortisone holds salt and water. Every one acts on the circulation, and not one is licensed for POTS, because no medicine anywhere is.

None goes near the tissue the circulation runs through. They are worth taking and nothing here is a reason to stop one. But they move the number on the monitor, and that was never the whole of it.

Then the shelf she built herself. Salt capsules and sachets, aimed at the fluid in the pipe, and the most recommended treatment for this condition on earth. It has never once been tested in a randomised trial. Compression, from outside the leg. Magnesium, for an excitable nerve. And a tube of gel.

The gel is worth stopping on, because if it says hyaluronic acid on the side she has bought this material once already. A molecule has to be under about 500 daltons to cross skin. Hyaluronan is far over that line. It was aimed at the outside of a shoulder and stayed there.

Which leaves one route anybody has tried. Swallowed, gut bacteria cut the long chain into fragments small enough to absorb, and the blood carries them into connective tissue. Not through the skin. Through the middle.

Hydraform Fascial Release is that material and nothing else. 150 mg of hyaluronan a day in two capsules, with the manganese the body uses to build connective tissue. Manganese contributes to the normal formation of connective tissue.

Not a painkiller, not an antidepressant, not a hormone. It does nothing to heart rate.

Evidence Peer-Reviewed Findings
03 What The Rate Drugs Actually Deliver
A 2026 review pooled nine studies in 245 patients taking ivabradine. It lowered standing heart rate by six to thirteen beats a minute. The authors' own conclusion: small, mostly observational, still waiting on randomised trials.
Kumar et al., Journal of Cardiovascular Pharmacology, 2026
Molecule size against the skin barrier.
About 500 daltons is all skin lets through. Hyaluronan is far over it, so the tube on the bathroom shelf never reached anything.

THE REAL-WORLD RESULTS, FROM REAL WOMEN

Connective tissue is not a side issue in POTS. In the one study to apply the full checklist, roughly a third of POTS patients met the criteria for hypermobile Ehlers-Danlos syndrome.

What is not known is whether this material does anything in POTS, because no trial of it has ever been run in POTS. Nobody has imaged fascia in these women and nobody has measured this molecule in them, and the standing measurement above is one small study. The dose comes from trials in other conditions.

What there is, is the people already taking it.

HOW EACH INGREDIENT SUPPORTS THE FASCIA

Hyaluronan, 150 mg a day. The molecule the sliding layers run on. It holds the film of water between them in place, and drinking more water does not put that film there.

Manganese. The mineral the body uses to build connective tissue, and the one ingredient here carrying a claim anybody is entitled to make.

No painkiller. No antidepressant. No stimulant, no salt load, nothing that touches heart rate. No collagen, which is the fibre and a different part of the tissue.

Vegan, third-party tested, made in a GMP-certified facility, sixty capsules to a pouch.

150 mg a day with manganese, at the kitchen counter. Nothing to time and nothing to log.

SAFE WITH YOUR CURRENT MEDICATIONS

Nothing needs to be stopped in order to start. No painkiller, no antidepressant, no stimulant, nothing that acts on heart rate.

It has not been tested alongside propranolol, ivabradine, midodrine or fludrocortisone, so it is worth a word with a doctor or pharmacist, particularly alongside a blood thinner. Nothing here is a reason to change what a cardiologist wrote.

Two capsules a day with water, with food or without. Nothing to time, nothing to log.

WHY STARTING SOONER MATTERS

Timing matters here, and not because of stock or a deadline.

A sliding surface under-supplied since a virus eleven years ago is further along than one under-supplied since last winter. It does not rehydrate on its own and left alone it only goes the same way.

The state the tissue is in today is the easiest it will ever be to supply.

And nothing happens in week one. Probably nothing in week three. Connective tissue turns over slowly, and anything promising a result by a date is selling the date.

90 day money back guarantee

Ninety days. If nothing changes, you keep the pouch and take the money back.

Take it for the full ninety days. If standing at the sink long enough to wash your own hair is no easier at the end of it, you get every penny back.

Nothing to post. No form. No conditions. Ninety days because that is how long this tissue takes.

I stopped planning my day around the stairs

I had got to the point where I worked out my whole morning around how many times I would have to stand up

Shower sitting down, dry my hair sitting down, breakfast sitting down. I started these in March and the thing I noticed first was not the heart rate, it was that I stopped greying out at the kitchen counter. I still pace myself. I just do not plan around it the way I did.

Mornings were always the worst of it and the afternoons were when I paid for the morning

Six or seven weeks in I realised I had got through a whole afternoon without lying down. My consultant has not changed anything I take. This is the only thing I added.

I can stand long enough to cook again

Standing at a hob for twenty minutes was genuinely beyond me

I would start a meal and have to leave it. I am not going to pretend it is gone, because it is not. But I cooked a Sunday dinner last month start to finish and that has not happened in four years.

QUESTIONS BEFORE YOU START

Do I have to stop my propranolol or ivabradine?

No. This contains no painkiller, no antidepressant, no stimulant and nothing that acts on heart rate, so there is nothing to come off first. It has not been formally tested alongside them, so check with your doctor or pharmacist, particularly alongside a blood thinner. Nothing here is a reason to change a prescription.

Will it bring my heart rate down?

No, and anyone who says otherwise is guessing. It does nothing to heart rate. It is aimed at the connective tissue that wraps every muscle, which is where the muscle pain, the neck and shoulder ache and the standing stiffness come from, and which nothing on the POTS shelf has been aimed at.

Is this the same as the hyaluronic acid cream I already have?

Same material, and that is exactly why the cream did nothing. A molecule has to be under roughly 500 daltons to cross intact skin, and hyaluronan is orders of magnitude larger. It was aimed at the outside of a shoulder and stayed there. Swallowing it is the only other route anybody has tried.

Has it been tested in POTS?

No. No trial of this material has ever been run in POTS, nobody has imaged fascia in these patients, and the dose comes from trials in other conditions. What can be said is that roughly a third of POTS patients meet the criteria for a connective tissue disorder, that fascia carries pain nerves in large numbers, and that those layers slide on this molecule.

Why ninety days and not thirty?

Because connective tissue turns over slowly and thirty days is not a fair test of it. A thirty day guarantee on something like this runs out before the question can be answered.

What happens if nothing changes?

Ninety days from the day it arrives, every penny back. Nothing to post, no form to fill in, no reason to give. Keep the pouch.

My sister was told for a year that there was nothing to find, and afterwards that there was nothing more to do. The people telling her so were measuring the wrong part of her.

That is not an apology anybody has offered her. It is why this page exists.
